UCSB requirements for processing stipends:

In order to process your stipends for attending the July UCSB Spatial Analysis workshop and have your checks issued by the last day of the session, it will be necessary for me to have the following information that is required by the university:

For Everyone:
1. Your home address (please update it if it is different from what you entered on your application or if you provided your university address)
2. Your social security number (US citizens)
   OR
   tax ID number (foreigners)
3. A signed waiver of liability
   
   and for non-US citizens only:
   
4. A filled in and signed Certificate of Academic Activity
5. A W8-BEN form filled out
6. Xeroxed copies of your passport picture page and visa page

Please provide me with this information and the completed forms that apply to you as soon as possible, and no later than Monday, June 20th to ensure that your check can be issued on time.
